What To Bring To Your Consultation
To help Dr. Onn assess your heart efficiently, please bring the following to your appointment if available.
Your Medications
Bring the medications themselves: the actual bottles, boxes or blister packs, not just a written list. Include everything you take: prescription medicines, supplements, and any traditional or natural remedies, so Dr. Onn can see exactly what and how much you are taking.
Cardiac Reports
Any existing angiogram, echocardiogram (echo), stress test, CT or cardiac MRI reports. Bring CDs or digital copies of imaging if you have them.
Previous Medical Reports
Specialist letters, discharge summaries or clinic reports relating to your heart condition, including from other hospitals.
Identification & Insurance
Your MyKad (or passport), and your medical insurance card or guarantee letter details if you are using insurance.
